Brentwood Hospital in Monroe, LA is seeking a dynamic Counselor / Social Worker to join our team dedicated to improving mental health. This outpatient role supports IOP/PHP, conducts psychosocial assessments, provides group and family therapies, and develops discharge plans.

Part-time hours: Monday–Friday, 3:00 PM–8:00 PM, with in-person and virtual options. The ideal candidate holds an LPC or LCSW license, a Master’s degree in social work or counseling, and experience in inpatient or outpatient

Qualifications

Master’s degree in social work or counseling.
LPC or LCSW licensure required.
Experience in inpatient or outpatient mental health settings preferred.

Responsibilities

Schedules appointments for in-person and virtual visits.
Conducts psychosocial assessments to identify treatment focus.
Provides group and family therapies using multiple modalities.
Documents daily progress notes and tracks progress toward goals.
Develops individualized discharge plans and coordinates aftercare resources.

Essential Job Functions

Schedule appointments for in person and virtual appointments.
Conducts psychosocial assessments to document history and identify preliminary issues for treatment focus by interviewing the patient and their family members.
Provides group and family process-oriented therapies as well as conducts educational and other didactic groups for patients and family members using various professional treatment modalities.
Daily progress notes summarized patient’s progress toward treatment goals; Contact with family, significant others, community agencies recorded in progress notes.
Develops and coordinates an individualized discharge plan for the patient by utilizing treatment team and chart information to determine the patient’s aftercare needs. Identifies and assesses family or community resources such as, group homes, or mental health practitioners to meet patients’ aftercare needs.
