Job summary

Speech Language Pathologists in outpatient care at Tidelands Health assess and treat communication and swallowing disorders in patients of all ages, using language and swallowing therapies and cognitive techniques. They develop treatment plans, document progress, and collaborate with an interdisciplinary team.

The role emphasizes accurate evaluation, effective treatment planning, and patient and family instruction to facilitate home-based care and ongoing improvement.

Qualifications

• Doctoral Degree of an accredited therapy program.
• Performed the required clinical internships satisfactorily.
• Ability to utilize therapy equipment in outpatient rehabilitation setting to provide treatment and procedures.

Responsibilities

• Reviews physician's referral (prescription), patient's condition, medical history, and submitted medical records to determine speech therapy treatment required
• Evaluate patients' speech and or swallowing through various standardized tests and clinical observations and evaluation and records findings to develop treatment programs
• Develop a treatment plan for patients with specific goals and tasks
• Plans and prepares written treatment program based on evaluation of patient and incorporate proper interventions that can help patients’ speech and or swallow deficits
• Evaluates effects of treatment at various stages and adjusts treatments to achieve maximum benefit
• Records treatment, responses, and progress in the patient’s electronic medical record and prepare reports as indicated by licensure, the provider, and or insurance recommendations
• Instructs patient and family in treatment procedures to be continued at home
• Completes documentation per clinical / department guidelines and meets expectations of monthly department chart audits
