Qualifications

• 1+ years of professional nursing experience.
• 1 year of private practice experience.
• Virginia RN license.
• BLS certification.
• Experience in outpatient care and internal medicine preferred.
• Experience with electronic health records and outpatient procedures.

Responsibilities

• Provide patient-centered nursing care to support preventive health and management of acute and chronic conditions.
• Collaborate with physicians and other healthcare professionals to coordinate comprehensive patient care.
• Perform clinical assessments, symptom triage, patient follow-up, and timely responses to patient inquiries.
• Support continuity of care through post-hospitalization, post-procedure, and other follow-up communications.
• Conduct nurse visits and assist with clinical procedures, diagnostic testing, injections, and patient education as appropriate.
• Support the use of clinical monitoring devices and technologies, and educate patients on their use.
