Job summary

Qualifications

Responsibilities

• Provide direct patient care in an outpatient setting according to the nursing plan.
• Develop and update patient care plans with the care team and family as needed.
• Perform data collection, assessments and documentation per policy.
• Collaborate with physicians and interdisciplinary teams to ensure quality outcomes.
• Support discharge planning and referrals to ensure continuity of care.
