Job summary

Trinity Health IHA Medical Group in Michigan seeks an compassionate Registered Nurse (RN) to join our outpatient care team. You will triage patients by phone and in-office visits, provide clinical guidance, education, and care coordination to improve outcomes and patient experience.

We value collaboration with providers, care managers, pharmacists, and behavioral health specialists, while managing refills, test results, and referrals.

Qualifications

• Graduate of an accredited Registered Nurse program.
• Valid Michigan RN license and CPR certification.
• 1–2 years of clinical experience in outpatient/physician office preferred.

Responsibilities

• Assess and triage patient needs by phone and in-office visits.
• Provide clinical guidance, patient education, and care coordination.
• Support patients through transitions of care and post-discharge follow-up.
• Collaborate with providers, care managers, pharmacists, behavioral health specialists, and other partners.
• Manage medication refills, test results, referrals, and prior authorizations.
• Identify preventive care needs and support chronic disease management.
• Help patients access community resources and navigate barriers to care.
• Serve as a clinical resource and leader within the care team.
