Job summary

Silver Creek Physical Therapy, a member of Therapy Partners Group, seeks an Outpatient Physical Therapist to join our dynamic team in California. You will deliver high-quality patient care, develop tailored treatment plans, and help patients achieve their rehabilitation goals in an outpatient setting.

The ideal candidate will hold a DPT and be licensed or eligible for licensure in California, with outpatient experience preferred.

Qualifications

• DPT from an accredited program.
• Licensed or eligible for licensure in CA.
• Outpatient experience preferred but not required.
• Ability to work collaboratively in a team.
• Commitment to exceptional patient care.

Responsibilities

• Patient Evaluation: Assess patients’ physical conditions and functional abilities.
• Treatment Planning: Create individualized plans based on evaluations and goals.
• Therapeutic Interventions: Apply manual therapy, exercise prescriptions, modalities, and education.
• Documentation: Record progress and outcomes in Raintree per clinic policies.
• Patient Education: Explain rehab processes and home programs to patients and families.
• Progress Monitoring: Reassess and adjust plans as needed.
• Professional Development: Stay updated with latest research and best practices.
