Job summary

SnapCare is seeking an Outpatient Physical Therapist for a 13-week contract in Doylestown, NC. The role requires a PT degree, active CPR certification, and at least 1 year of outpatient experience with strong orthopedic knowledge.

Qualifications

• Minimum 1 year of outpatient clinical experience.
• Degree in Physical Therapy from an accredited school.
• CPR Certification required.
• Strong orthopedic knowledge required.

Responsibilities

• Evaluate assigned patients and develop, implement, and modify individualized treatment plans based on patient needs, goals, and physician direction.
• Delegate and direct all discipline-specific therapy services for assigned patients.
• Design functional treatment programs to support effective return to community activities.
• Complete comprehensive medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry.
• Ensure all care is provided under signed, valid physician orders and within insurance approval.
• Adhere to discipline-specific state practice act and maintain professional licensure in good standing.
• Participate in student program as assigned, providing a positive learning environment with appropriate supervision.
• Actively pursue professional development through continuing education, self-study, and involvement in professional activities.
• Follow all clinic guidelines outlining standards of personal appearance, attendance, and punctuality.
