Job summary

Centra Healthcare Solutions seeks an experienced or entry-level Physical Therapist (PT) for a contract assignment in Red Bluff, CA. The PT will assess referred patients, create goals, and implement treatment programs under physician direction, with documentation of outcomes and patient education.

The role requires graduation from an accredited PT program, an active or in-process state license, and CPR certification. Collaborates with the care team to meet patient rehabilitation goals.

Qualifications

• Graduate from an accredited PT program.
• Current state PT license or in licensure process.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services per physician orders.
• Collaborate with multidisciplinary team to plan and evaluate goals.

Responsibilities
