Job summary

Silver Creek Physical Therapy, part of Therapy Partners Group, seeks a dedicated Outpatient Physical Therapist Assistant to join our team serving San Jose and surrounding communities for over 20 years.

Responsibilities include assisting with patient follow-up, treatment planning, and therapeutic interventions under supervision, and documenting progress in the chart while educating patients and families. Collaboration with the healthcare team is essential to ensure exceptional patient care.

Qualifications

• Associate degree in PTA and active licensure or eligibility required.
• Outpatient experience preferred but not required.
• Strong teamwork and commitment to patient-centered care.

Responsibilities

• Assist with patient follow-up and exercise modalities.
• Help develop and implement patient treatment plans.
• Perform therapeutic interventions including manual therapy and education.
• Document progress in the chart and report changes.
• Educate patients and families on exercises and pain management.
• Collaborate with the healthcare team to ensure safe care.
