Job summary

Parkview Health in Indiana seeks a licensed Physical Therapist to evaluate, plan, and implement therapy, and to document treatment outcomes. The role involves developing individualized plans, supervising Physical Therapist Assistants, and educating patients and families for post-discharge care.

Must be licensed in Indiana (or eligible), CPR certified (or within 90 days), and capable of lifting substantial loads; Doctorate of Physical Therapy preferred for post-2016 graduates.

Qualifications

  • Must be a graduate of an accredited school of physical therapy with a Doctorate of Physical Therapy preferred for graduates after 2016; Bachelor’s degree prior to 2016 accepted.
  • Must hold or be eligible for Physical Therapist license in the state where services are provided; CPR certification required or obtained within 90 days.
  • American Physical Therapy Association membership preferred.
  • Ability to lift 50 pounds, assist with 100 pounds, and push up to 300 pounds; willing to work at other locations as assigned.

  • Provides professional patient care and practices physical therapy by evaluating, planning, implementing, and documenting treatment.
  • Develops and implements individualized treatment plans based on patient needs with achievable functional goals; revises plans as needed.
  • Instructs and collaborates with Physical Therapist Assistants, supervising patient treatment and procedures.
  • Educates patients, families and caregivers in exercises and home programs for discharge and ongoing care.
  • Completes documentation and billing in a timely manner in accordance with hospital, department, and regulatory standards.

Job description

Provides professional patient care and practices physical therapy by evaluating, planning, implementing, and documenting treatment. Evaluates and treats patients in accordance with the order from the physician and provides treatment through the use of a variety of therapeutic procedures and assistive devices intended to maximize functional potential in all areas of the patient’s life. Develops and implements an individualized treatment plan based on patient needs and containing achievable functional goals. Revises treatment plans, as needed, throughout the treatment process. Instructs and collaborates with Physical Therapist Assistants (PTA) regarding treatment procedures to be utilized and provides the appropriate supervision of patients being treated by the PTA. Establishes and educates patients, families and other caregivers in exercises and therapeutic home programs to be continued following discharge. Completes the appropriate documentation and billing in a timely manner. All care is provided in accordance with the philosophy and essential functions of the hospital, the rehabilitation department, the Indiana State Practice Act, the American Physical Therapy Association, and The Joint Commission.

Education

Must be a graduate of an accredited school of physical therapy, with a physical therapy degree as follows: Minimum of a Bachelor’s degree, if graduated prior 1/1/2016; Doctorate of Physical Therapy required, if graduated after 1/1/2016; International Candidates must be a graduate of an accredited school and possess a minimum of a Bachelor’s degree in Physical Therapy. Doctorate of Physical Therapy degree preferred.

Licensure/Certification

Must hold or be eligible to receive a Physical Therapist license in the State where services will be provided. State licensure must be obtained within the first 90 days of employment. Must be certified in CPR or must become certified within the first 90 days of employment. Must maintain all certifications and license according to the expiration date. American Physical Therapy Association membership preferred. If assigned to drive a company vehicle, a valid driver's license is required. If assigned to drive a personal vehicle, proof of insurance is required in addition to a valid driver's license.

Experience

Work experience preferred, but not required, other than clinical experience within educational training program.

Other Qualifications

Must be able to lift 50 pounds, assist with 100 pounds, may be required to push up to 300 pounds. Must be willing to work at other locations as assigned.
