Qualifications

• Active and unrestricted medical license or willingness to obtain license in the practicing state.
• Experience in outpatient primary care is required.
• Additional training in Family Medicine, Internal Medicine, or Geriatrics preferred.

Responsibilities

• Provide comprehensive outpatient primary care to patients.
• Collaborate with care team to coordinate preventive and chronic care.
• Participate in value-based care initiatives and quality metrics.
