Apex Home Health, Hospice & Home Care in Bridgeport is seeking an Outpatient Primary Care Provider to deliver comprehensive care to complex patients within an interdisciplinary team.

You will assess, exam, and coordinate care, ordering tests and developing individualized treatment plans while collaborating with physicians to ensure ongoing patient care and optimal patient outcomes.
