Responsibilities

• Implements patient care programs as outlined by the physical therapist, using appropriate techniques and modalities to ensure quality care and safety.
• Maintains at least weekly communication with supervising therapists for each patient to discuss treatment plans and changes.
• Completes patient documentation in accordance with departmental policies and procedures.
• Maintains communication with physicians and other health care professionals.
• Assists with discharge planning.

Education

Graduate of an accredited Physical Therapist Assistant program.

Certified or eligible for certification in the State of Kentucky.
