Community Health Systems is seeking a Physical Therapist for a full-time outpatient role in Carlsbad, New Mexico. You will evaluate, plan, and administer therapeutic procedures, establish goals, and document progress to support recovery.

You will collaborate with physicians and an interdisciplinary team, ensure safety and compliance, and maintain records. A PT license in NM and BLS within 30 days are required, with 0–2 years of experience preferred.

Qualifikationen

• Bachelor's Degree in Physical Therapy is required.
• 0–2 years of experience in a physical therapy setting is required.
• Licensed PT in the state of employment is required.
• BLS certification obtained within 30 days of hire.

• Conducts comprehensive PT evaluations and develops individualized treatment plans.
• Administers safe and effective therapy treatments and documents interventions.
• Reassesses and adjusts treatment plans based on patient progress.
• Documents evaluations, plans, progress notes and billing information.
• Educates patients and families on treatment plans and post-care.
• Operates therapy equipment safely and maintains a clean environment.
• Collaborates with referring physicians and interdisciplinary team for continuity of care.
• Maintains compliance with facility safety, infection control and risk management.
