Responsibilities

JOB SUMMARY

The Physical Therapist is responsible for the evaluation, administration, and oversight of evidence‑based physical therapy services provided to individuals with physical disabilities and/or functional limitations. They use specialized skills to promote prevention of dysfunction, restoration of function, and/or reversal of the progression of pathology to enhance the quality of movement and daily living skills. They develop and update appropriate plans of care in collaboration with the patient/family to assist in returning to prior levels of function. They act in accordance with physician referral guidelines, state and national standards, regulatory agencies, and departmental policies. They embody strong customer service skills and communicate concisely and tactfully with physicians, co‑workers, caregivers, family members, patients, and other healthcare providers.

Essential Functions

• Implements an individualized treatment plan through skilled, evidence‑based therapy techniques, including exercises, manual techniques, functional mobility training, ADL training, and modalities as indicated. Provides verbal and written instructions to patients, caregivers, and family members. Maintains knowledge of current therapy techniques through continuing education, journals, and regulatory requirements.
• Evaluates patients per the Department Operating Policy, synthesizing data into treatment diagnoses, establishing measurable and functional goals, and a defined discharge plan. Performs ongoing evaluation of patient response to ensure effective service provision, documented accordingly.
• Maintains accurate, current documentation in the patient’s medical record, including evaluations, daily treatment notes, treatment plans, progress reports, discharge summaries, and daily patient charges. Completes billing per department procedures and within set timeframes to support financial stewardship.
• Confers with interdisciplinary team members to clarify treatment plans and discuss patient status. Consults with medical personnel about needs, status, and discharge plans. Provides timely feedback to case managers and key personnel regarding discharge needs, recommended discharge levels of care, and equipment needs. Participates in interdisciplinary rounds as indicated. Communicates information and addresses concerns to achieve optimal outcomes, as supported by feedback and documentation.
• Delegates appropriate patients to physical therapist assistants, supervises care delivered by assistants and assistive personnel, and provides updated treatment plans and instruction. Serves as a resource for therapy‑related issues for assistive personnel and staff from other departments.
• Assists in identifying department needs and process improvements. Contributes to recruitment and retention of staff through positive role‑modeling and fostering peer relationships. Participates in departmental and interdepartmental meetings, in‑services, utilization review programs, committees, and student preceptor and mentor programs.

Education

• Bachelor's in Physical Therapy from an accredited university approved by the APTA – Required
• Master’s or Doctorate in Physical Therapy from an accredited university approved by the APTA – Preferred

Experience

• Other Network float position: 6 months recent experience – Preferred
• 6 months experience – Preferred

License and Certifications

• Basic Life Support (BLS) – Required
• Physical Therapist – license from the AZ State Board of Physical Therapy or PT Compact – Required (current license in good standing)
• American Physical Therapy Association (APTA) – Certificate – Preferred
