Qualifications

• Graduate of an APTA accredited program or equivalent.
• Licensed Physical Therapist.
• Excellent written and verbal communication skills.
• Proficient computer skills (MS Office/Google Suite).

Responsibilities

• Participates on committees as requested by Supervisor.
• Delivers the highest level of service to residents, families and visitors.
• Maintains safe environment and uses proper body mechanics during activities; completes resident treatments as scheduled.
• Performs discharge planning at evaluation and updates it through therapy.
• Assesses need for DME and ensures proper fitting of devices; educates resident/family on use.
