Job summary

Tidelands Health is seeking a PRN Physical Therapist for outpatient care in South Carolina. The role involves evaluating patients, developing and implementing treatment plans, and coordinating with a multidisciplinary team to maximize mobility and function in an outpatient setting.

The PT will administer therapeutic modalities, document progress, and educate patients and families for home programs. Prior SC licensure and knowledge of rehab programs are essential.

Qualifications

• Bachelor's Degree required.
• Master's Degree if obtained prior to 2016 preferred.
• Doctoral Degree if obtained after 2016 required.

Responsibilities

• Review physician referral, patient condition, medical history and records to determine PT treatment.
• Evaluate functional ability using standardized tests and document findings to guide therapy.
• Develop individualized treatment plans with specific goals and tasks.
• Administer PT modalities (e.g., electrical stimulation, ultrasound) as appropriate.
• Record treatment, responses and progress in the EMR per guidelines.
• Instruct patient and family in home exercise programs and post-discharge care.
• Educate referring physicians and staff about rehab programs and services.
• Strive to meet monthly productivity and quality metrics.
