Brooks Rehabilitation is seeking a compassionate Physical Therapist to join our Outpatient team in Jacksonville, FL. You will evaluate patients, design treatment plans, and contribute to a patient-centered program.

The role emphasizes high-quality care, collaboration with orthopedics and pelvic health specialists, and ongoing professional growth through mentorship and residency opportunities.

Qualificações

• Current Florida Physical Therapist license and CPR/BLS certification.
• Knowledge of federal and state professional requirements.
• Experience delivering outpatient PT care is preferred.

Responsabilidades

• Complete the new patient evaluation, establish customized treatment plans and patient goals, and participate in the implementation of patient care programs.
• Deliver high quality patient care for multiple rehabilitation diagnoses.
• Provide a positive patient experience through patient engagement and progressive treatment.
• Effectively communicate both verbally and written.
