Qualifications

• Current active Indiana Physical Therapy license.
• BLS certification within 90 days of hire.
• Ability to work 40 hours per week with occasional late or early shifts to accommodate patients.

Responsibilities

• Evaluate patients in the outpatient clinic and develop an appropriate treatment plan.
• Utilize physician and community resources and patient support systems.
• Perform skilled physical therapy procedures/assessments and educate patients and families.
• Document treatment and care using electronic records.
