Job summary

Southern New Hampshire Health in Nashua, NH seeks a dedicated Physical Therapist for outpatient rehab. The role is full-time, Monday–Friday with two evenings until 7:30 pm, providing evaluation, treatment planning, and education for diverse patients in a supportive, collaborative environment.

The candidate will hold a DPT, NH license or eligibility, and CPR certification, with outpatient PT experience preferred. Graston, lymphedema, and dry needling skills are valued. Competitive benefits apply.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current NH Physical Therapy license or eligibility to license.
• CPR Certification.
• Outpatient PT experience (1–2 years) preferred.
• Desirable: Graston Technique, Lymphedema Certification, Dry Needling.

Responsibilities

• Evaluate patients using subjective data, objective measures, and prognosis.
• Develop and adjust patient-specific treatment plans to improve function.
• Deliver patient and family education to maximize engagement.
• Document clinical findings and progress in the EMR timely.
• Complete required insurance and payer information.
• Provide supervision to new staff or affiliating students as assigned.
• Collaborate with physicians and care teams for coordinated care.
• Maintain competency and apply appropriate equipment and techniques.
• Deliver excellent customer service and ensure a safe patient experience.
