Fort Worth Physical Therapy, an H2 Health company, seeks a Physical Therapist to join our Grapevine, TX outpatient team. You will conduct evaluations, develop plans of care, and deliver evidence-based interventions to improve mobility, strength and balance in an outpatient setting.

Qualifications

• DPT or equivalent CAPTE-accredited program.
• Current Texas PT license or eligibility for licensure.
• CPR certification or willingness to obtain.

Responsibilities

• Perform comprehensive patient evaluations and develop individualized plans of care.
• Deliver evidence-based interventions to improve mobility, strength, balance and function.
• Treat orthopedic, post-operative, sports medicine, vestibular, balance and musculoskeletal conditions.
• Collaborate with patients, families, physicians and interdisciplinary team to achieve outcomes.
• Educate patients on injury prevention, wellness and home exercise programs.
• Maintain accurate documentation of progress and outcomes.
