Job summary

Qualifications

• Evaluates patient’s physical, cognitive, and functional status using standardized tests and measures.
• Incorporates pertinent medical information into the evaluation.
• Ages, language and cultural needs are incorporated into the individualized treatment plan.

Responsibilities

• Formulates individualized treatment plans consistent with objective findings and sets short- and long-term goals.
• Administers treatments and monitors patient responses.
• Supervises PT assistants, PT/PTA students, and techs.
• Maintains thorough, timely and legible documentation.
• Communicates with patients, families, physicians, and the therapy team.
• Participates in discharge planning to promote safe transitions.
• Ensures patient privacy and compliant practices.
• Keeps work area clean and participates in quality improvement efforts.
