Key Responsibilities

• Conduct thorough evaluations of patients by reviewing medical histories, performing specific assessments, and documenting findings accurately.
• Communicate evaluation results and treatment plans clearly to patients and their families, ensuring their involvement in setting achievable goals.
• Implement individualized treatment protocols based on comprehensive evaluation results, accounting for each patient’s unique condition and needs.
• Collaborate effectively with other healthcare professionals to ensure coordinated care and recognize when modifications to the treatment plan are required.
• Oversee and document patient discharge planning, ensuring that follow-up care and ongoing treatment recommendations are communicated clearly and timely.
