Qualifications

• Degree in Physical Therapy from an accredited school is required.
• Active and applicable state license required for the state where work will be performed.
• CPR certification required.

Responsibilities

• Evaluate assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction.
• Responsible for delegation and direction of all discipline-specific therapy services for assigned patients.
• Design functional treatment programs to support effective return to community activities.
• Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry.
• Ensure care is provided under signed, valid physician orders and within insurance approval.
• Participate in student programs as assigned, providing appropriate supervision and completing student evaluations.
• Adhere to discipline-specific State Practice Act and all other applicable regulatory agency requirements.
• Maintain professional licensure in good standing and complete all state-mandated continuing education requirements.
