Job summary

Qualifications

• Graduate of an accredited PT program (Required).
• Licensed Physical Therapist in Ohio and/or Multi-State Compact license (Required).
• Ability to evaluate and treat patients across developmental ages (Required).

Responsibilities

• Performs evaluation(s) and develops an effective treatment plan appropriate to a patient’s rehabilitation needs.
• Educates patient/family/caregiver in applicable health information, treatment and prevention.
• Initiates and participates in the discharge planning process.
• Performs other department activities such as documentation, billing, and support of department operations.
• Performs other duties as assigned.
• Complies with all policies and standards.
• Maintains PHI securely per UH policies and completes annual PHI training.
