Qualifications

• Graduate of an accredited physical therapy program and licensure in Wisconsin.
• APTA Certification Exam completion required; valid state license.
• CPR/First Aid certifications or obtainable within 6 months.

• Proficiency with Microsoft Office; experience with EHR preferred.

Responsibilities

• Evaluate and treat residents per physician referral.
• Develop and implement therapy plans and progress notes.
• Provide resident and family education and counseling as needed.
• Collaborate with interdisciplinary team to coordinate care.
• Maintain documentation and ensure safety and quality standards.
