Qualifications

• Doctorate degree in Physical Therapy from an accredited school.
• Maintains continuing education requirements as mandated by applicable state standards.
• Licensed Physical Therapist in the State where you work.
• Basic Life Support (BLS) for Healthcare Providers within 90 days.

Responsibilities

• Interprets physician referrals and completes musculoskeletal, neuromuscular, cardiopulmonary, and integumentary evaluations.
• Develops and implements plan of care based on evaluation results and goals; monitors outcomes.
• Discharges patients with written home programs and referrals; coordinates transitions of care.
• Completes required documentation and billing per department policies; maintains productivity.
• Maintains patient safety and adheres to infection control measures across modalities.
