Job summary

Apex Home Health, Hospice & Home Care seeks a Licensed Physical Therapist to join the outpatient rehabilitation team. You will assess patients, design individualized treatment plans, and deliver evidence-based therapy to restore function and mobility.

Work with a multidisciplinary team to optimize patient outcomes, educate families on exercises and self-management, and document progress and findings accurately while contributing to a patient-first, community-minded culture.
