Job summary

Personal Touch Home Health Care Services Inc. is seeking a Physical Therapist for outpatient care focusing on orthopedic, neurologic, and post-surgical rehabilitation. You will assess patients, design individualized treatment plans, and deliver therapeutic exercises and manual therapy to improve function and reduce pain.

You will document patient progress in the EMR system and collaborate with physicians and care teams to optimize outcomes, contributing to high-quality community health services.

Qualifications
