Northshore Home Health Care is seeking an Outpatient Physical Therapist to provide evidence-based evaluation and treatment for adolescents and adults. You will conduct thorough assessments, develop individualized plans, and deliver hands-on as well as exercise-based interventions to enhance function and quality of life.

The role emphasizes education for patients and families and close collaboration with physicians, nurses, and rehabilitation colleagues in a dynamic, inclusive environment.
