Qualifications

• Must have Bachelor’s Degree in Physical Therapy.
• Licensed Physical Therapist in the State of Illinois.
• BLS certification from American Heart Association upon hire.

Responsibilities

• Provides direct patient care and participates in therapy programs.
• Teaches and participates in committees and process improvement.
• Serves as a role model for therapists and clinical interns.
• Maintains patient plans of care with PT/PTA collaboration.
• Documents notes in EMR and completes daily notes as required.
• Delivers educational materials tailored to patient understanding.
• Participates in department projects and student programs.
• Flexes schedule to meet patient care needs.
