• Monitors, examines, and evaluates patient conditions and assists with the care plan.
• Delivers care per plan and communicates with the care team to coordinate patient flow.
• Advocates for patients and families, providing education and ensuring confidentiality and safety.
• Supports performance improvement initiatives and professional growth.
• Mentors and supports peers to foster a positive, professional work environment.
• Performs other duties as assigned and adheres to Wellstar policies.
