Job summary

Telos Health Systems, Inc. seeks a licensed Occupational Therapist (OT) for Outpatient care in Ormond Beach, FL. This PRN role focuses on evaluating residents, creating personalized treatment plans, and delivering therapy in senior living environments.

Strong documentation and communication with families are essential. Qualified candidates hold a Florida OT license, CPR certification, and have experience with ADLs, fine motor skills, and cognitive training.

Qualifications

• Bachelor’s, Master’s or Doctorate in Occupational Therapy from an accredited program.
• Current Occupational Therapist license in Florida.
• CPR certification (per facility/state requirements).
• Strong interpersonal, clinical and documentation skills.

Responsibilities

• Conduct evaluations and develop customized occupational therapy treatment plans.
• Deliver direct therapy to residents in a senior living setting.
• Improve ADLs, fine motor skills, and cognitive function.
• Educate residents, families, and caregivers on goals and strategies.
• Document progress and adjust treatment plans as needed.
• Collaborate with interdisciplinary care teams to ensure integrated care.

Education

Bachelor’s, Master’s, or Doctorate degree in Occupational Therapy

Current Florida OT license
