Qualifications

• Professional demeanor and strong interpersonal and communication skills.
• Ability to protect privacy and PHI of patients and caregivers.

Responsibilities

• Provide patient care under clinician supervision, gathering information relevant to visits and conditions.
• Communicate clearly with patients via telephone and in person.
• Collaborate with cross-functional teams to support patient care and follow-through.
