Job summary

OhioHealth Mansfield seeks a dedicated LPN to provide nursing care to patients across the health-illness continuum in diverse settings, under the guidance of the healthcare team. The LPN will document and monitor patient status and delegate tasks within scope of practice.

The successful candidate will hold an active Ohio LPN license, BLS certification, and demonstrate strong teamwork, communication, and patient advocacy. Day shift, 40 hours per week; no supervisory responsibilities.

Qualifications

• High School or GED (Required).
• BLS - Basic Life Support (American Heart Association)
• LPN - Licensed Practical Nurse licensure required.

Responsibilities

• Performs initial, ongoing and functional health status assessment.
• Documents and communicates assessment findings to appropriate team members.
• Delegates tasks according to criteria and state regulations; coordinates continuity of care.
• Participates in process improvement and advocates for patients.
