Job summary

The University of Rochester in Rochester, NY is seeking an LPN to join the Primary Care Network team in an outpatient setting. You will provide direct care to adult patients (18+) and support the care delivery system within a collaborative, multidisciplinary team.

Role involves administering medications and vaccines, basic lab tasks, and EMR documentation. A strong team focus and willingness to work weekdays is expected.

Qualifications

• Self-starter and team-oriented; enjoys collaborating with others.
• 6 months in direct nursing care preferred but not required.

Responsibilities

• Provide direct patient care in an outpatient setting.
• Contribute to unit activities and support improvements in care delivery.
• Assist other staff in implementing patient care as a team member.
• Provide care for assigned patients under supervision of RN or MD.
• Participate in plan of care, perform nursing interventions, and educate patients and families.
• Work with Ambulatory Techs, RNs, Physicians and APPs as part of the team.
• Administer medications/vaccines, conduct minor lab testing, and communicate with patients and providers using EMR.
