Qualifications

• Minimum of 1500 hours of acute care or infusion pharmacy experience within the last 3 years.
• Degree in Pharmacy meeting minimum education requirements for licensure.
• California Pharmacist License required at hire.
• Controlled Substance Utilization Review and Evaluation System Registration required at hire.
• National Provider Identifier required at hire.
• Basic Life Support within 3 months of hire.

Responsibilities

• Upholds Kaiser Permanente policies and procedures, and applicable regulations.
• Performs sterile and non-sterile compounding, calculations, and dosing per protocol.
• Reviews and monitors IV therapy for home care, LTC, hospice and outpatient settings.
• Communicates with patients, caregivers, and health-care team to optimize care.
• Reviews orders for appropriateness and intervenes when needed.
• Documents information in the medical record per policy.
