Qualifications

• Board-Certified or Board-Eligible in Internal Medicine with a completed Geriatric Medicine Fellowship.
• Focus on outpatient geriatric care in an integrated healthcare system.
• Ability to coordinate care with referring physicians and specialists.

Responsibilities

• Provide comprehensive outpatient care for older adults with emphasis on preventive medicine.
• Manage chronic diseases and complex medical conditions in geriatric patients.
• Conduct comprehensive geriatric assessments and address cognitive/functional health concerns.
• Collaborate with multidisciplinary teams to optimize patient outcomes.
• Develop long-term patient relationships and coordinate with primary and specialty care.
