Job summary

Sycamore Physician Contracting LLC is seeking a Primary Care Physician for a permanent outpatient role in Oklahoma. The practice emphasizes senior care, preventive services, and quality outcomes with a relationship-based care model supported by clinical, pharmacy, and administrative teams.

No night or weekend obligations and no hospital rounding. Board-certified or eligible physicians with Oklahoma licensure are preferred, with additional emphasis on Medicare Advantage or value-based care

Qualifications

• Outpatient primary care focus for seniors and Medicare Advantage patients.
• Board certification or eligibility in Family Medicine, Internal Medicine, or Geriatrics.
• Active Oklahoma medical license or ability to obtain license.

Responsibilities

• Provide outpatient primary care to seniors and Medicare Advantage patients.
• Coordinate care with clinical, pharmacy, care-coordination, and administrative teams.
• Manage chronic conditions including diabetes, hypertension, COPD, heart failure, and CKD.
• Deliver preventive care and drive quality outcomes.
