Job summary

Trinity Health Mid-Atlantic Medical Group in Darby, PA is seeking a Board-Certified/Board-Eligible Family Medicine or Internal Medicine physician to support primary care expansion and access initiatives across the Pennsylvania market.

This 100% outpatient role features a high-volume practice, team-based care, Epic EMR, minimal on-call, and a compensation package with comprehensive benefits.

Qualifications

• Must be Board-certified orBoard-eligible in Family Medicine or Internal Medicine.
• Outpatient focused practice with collaboration across care team.
• Experience with Epic EMR is a plus.

Responsibilities

• Diagnose and manage complex chronic diseases.
• Address Social Determinants of Health (SDOH) barriers to care.
• Provide trauma-informed care to patients in high-stress settings.
• Coordinate with on-site social workers, case managers, and behavioral health specialists.
• Administer preventive immunizations, screenings, and harm reduction strategies.
• Navigate financial barriers using sliding fee scales and formulary options.
• Document patient encounters accurately in the EHR.
