Job summary

Carle Health is seeking a Dietitian to provide nutrition education and counseling to clinic patients and groups. The role assesses outpatient nutrition needs, develops and implements a nutrition care plan, and supports public programs and professional staff through education materials.

The position requires Licensed Dietitian Nutritionist in Illinois and RD certification, plus a Bachelor's in a related field.

Qualifications

• Certifications: Licensed Dietitian Nutritionist - IDFPR; Registered Dietitian - CDR.
• Education: Bachelor's Degree: Related Field (Required).

Responsibilities

• Provide nutrition services and medical nutrition therapy integrated with the patient's medical goals.
• Identify and provide age specific nutrition counseling to meet the cultural needs of the patients and their families.
• Evaluate, interpret, monitor and document the nutritional status and nutritional needs of patients using established standards of care and practice guidelines.
• Develop, review, update and implement educational materials to meet the needs of patients and professionals.
• Participates in the scheduled test tray program.
• Provides learning experience for dietetic interns as outlined in their program.
• Assists with in-service training for diet office personnel and staff.
• Assists in the development of patient education materials.
• Provides continual monitoring of patients on assigned nursing units to ensure quality of nutrition care services.
