Job summary

Kaiser Foundation Health Plan of Washington is seeking a Care Manager Liaison Nurse for outpatient complex care in Spokane. The role combines clinic-based case management three days a week with telecommuting on other days, focusing on optimal patient outcomes and resource utilization.

You will work closely with primary and specialty care teams to develop collaborative care plans, manage medications for chronic conditions, support patient self-management, and facilitate seamless transitions

Qualifications

• Minimum three (3) years of RN medical/surgical/ambulatory clinical experience.
• Minimum two (2) years of RN experience in ambulatory case management, care coordination or disease management.
• Bachelor's degree.
• Registered Nurse License (Washington) required at hire OR Compact License: Registered Nurse required at hire
• Basic Life Support required at hire
• Case Manager Certificate within 36 months of hire

Responsibilities

• Ensures patients referred to case management meet established case management criteria and develops patient-centered plans with goals and outcomes.
• Collaborates with other health care professionals to monitor progress and adapt plans; documents interventions and maintains communication with care team.
• Directs and guides the plan of care to ensure seamless continuum of care and facilitates referrals as needed.
