Job summary

Eleve Wellness in New Jersey is seeking a compassionate Clinical Therapist to join our outpatient behavioral health team. This full-time, in-person role handles a primary caseload with individual, group, and family therapy and requires NJ licensure in good standing.

We welcome both independently licensed and associate-level clinicians, with supervision provided for associates. Candidates experienced in substance use, co-occurring disorders, and trauma-informed care are preferred.

Qualifications

• Active New Jersey professional license or credential in good standing.
• Supervised practice for associate-level clinicians per NJ requirements.
• Experience in substance use disorder treatment, co-occurring disorders, or outpatient behavioral health is preferred.

Responsibilities

• Provide individual, group, and family therapy as clinically appropriate.
• Serve as Primary Therapist for an assigned caseload.
• Complete biopsychosocial and clinical assessments.
• Develop individualized, measurable treatment plans.
• Document progress notes, treatment plans, assessments, discharge docs on time.
• Assess progress, barriers, relapse risk, and continued needs.
• Participate in treatment team meetings and case conferences.
• Coordinate care with medical, psychiatric, case management and others.
• Complete discharge and aftercare planning with required documentation.
• Maintain accurate, compliant clinical records in the EHR.
• Conduct risk and safety assessments and follow escalation procedures.
• Communicate changes in client condition to the team and leadership.
• Participate in chart audits, utilization reviews, and QA processes.
• Support clients with evidence-based and person-centered interventions.
• Maintain professional, ethical, regulatory standards for outpatient services.
• Manage caseload consistent with program census and licensure requirements.
• Maintain reliable attendance and caseload coverage.
