Heartshare seeks a Licensed Professional (OLP) to deliver community-based behavioral health services to children and families in homes, schools, and other settings across Brooklyn and Queens. The role involves psychosocial assessments, treatment planning, and psychotherapy, with care coordination in the community.
The position requires a Master's in Mental Health Counseling or Social Work and NYS licensure (LCSW/LMSW/LMHC/MHC-LP), plus flexibility for home visits throughout the service area.
The Licensed Professional (Other Licensed Practitioner – OLP) is a field-based clinician who provides comprehensive behavioral health services directly within the communities where children and families live, attend school, and engage in daily life. Rather than providing traditional office-based therapy, the Licensed Professional travels throughout the community to deliver services in homes, schools, and other natural settings, allowing treatment to occur in the environments where challenges arise and skills can be practiced in real time.
The OLP is responsible for conducting comprehensive psychosocial assessments, establishing diagnoses and determining medical necessity, developing individualized treatment plans, providing individual, family, and group psychotherapy, delivering crisis intervention, and coordinating care with families and community providers. This role requires flexibility, strong clinical judgment, and the ability to independently manage a community-based caseload while collaborating closely with caregivers, schools, medical providers, and multidisciplinary team members. The overall goal is to improve emotional and behavioral functioning, strengthen family relationships, promote stability across home, school, and community settings, and reduce the need for higher levels of behavioral health intervention.
