Job summary

Achieve Therapy & Fitness in Grand Forks, ND is hiring a Licensed Occupational Therapist to join our outpatient and home health teams. You will perform comprehensive evaluations, create individualized treatment plans, and help patients regain independence through evidence-based care.

Qualifications

• Must be a graduate of an accredited Occupational Therapy program.
• Current Occupational Therapist license (or license eligible) in Minnesota and/or North Dakota.
• Current CPR certification.
• Valid driver’s license and reliable transportation for home health visits.
• Experience in outpatient orthopedics and/or home health preferred.

Responsibilities

• Perform comprehensive occupational therapy evaluations.
• Develop individualized treatment plans reflecting patient goals and priorities.
• Treat orthopedic, neurological, upper extremity, balance, cognitive, and functional conditions.
• Provide home health occupational therapy services within the local community.
• Educate patients and caregivers to promote confidence and long-term success.
• Collaborate with physicians, therapists, and healthcare providers to ensure continuity of care.
• Maintain timely and accurate clinical documentation.
• Participate in team meetings, education opportunities, and quality improvement initiatives.
• Build meaningful patient relationships through trust, empathy, and encouragement.
