Job summary

Adoration Health is seeking a Home Health Occupational Therapist to provide 1:1 patient care in Providence, RI. You will evaluate and treat patients in their homes, coordinate with a multidisciplinary team, and help patients achieve independence with adaptive equipment and personalized goals.

The role offers a supportive environment, opportunities for growth, and a PRN schedule. Applicants should have CAPTE-approved OT education, state licensure, CPR, and prior adult OT/home health experience.

Qualifications

• Graduate of a CAPTE-approved Occupational Therapy Education program.
• Current OT licensure in the state and CPR certification.
• Two years of experience in adult occupational therapy; home health experience preferred.

Responsibilities

• Perform OT evaluation and treatment per physician orders, including adaptive equipment.
• Establish goals and treatment plans to maximize independence and function.
• Maintain communication with treatment team, physician, patient, and family.
• Maintain thorough documentation for proper billing.
• Supervise OT Assistants in patient care per state act.

Responsibilities

As a Home Health Occupational Therapist , You w ill:

• P erform occupational therapy evaluation and treatment of patients based upon physician orders, including recommendation and procurement of adaptive equipment
• Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function
• Maintain communication among the treatment team, physician, patient, and family members as applicable
• Maintain thorough and timely documentation of services provided to ensure proper billing
• Supervise Occupational Therapy Assistants in direct patient care and patient-related activities in accordance with state practice act

Qualifications

• Graduate of a n Occupational Therapy Education program approved by CAPTE
• Current Occupational Therapist licensure in State and American Heart Association CPR certification required ; CPR certification may be offered and completed upon hire
• Two (2) years of experience in adult occupationa l therapy, home health experience preferred
• Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; physical therapy skills per competency checklist
