Job summary

Personal Touch Home Care seeks a licensed Occupational Therapist to deliver home-based rehabilitation in Queens, NY. This fee-for-service, 1099 role covers Astoria, Springfield Gardens, Jamaica, and Flushing with flexible daytime and evening cases.

You will assess patients, design and implement treatment plans, and help clients regain daily living skills in their own homes. Pay ranges from $75 to $100 per visit, with a supportive leadership team and opportunities to work with pediatric, adult,

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid NY license/registration.
• Home health experience preferred.

Responsibilities

• Assess referred patients in their home and develop rehabilitation plans.
• Provide direct patient care to improve daily living and work skills.
• Implement and monitor treatment plans under physician guidance.
• Document patient progress and discharge when appropriate.
• Educate patients and families on therapy methods and home exercises.

This is fee-for-service position, paid per visit and this is a 1099, contracted position. In order to be considered for this position, you MUST have a valid and active license in the state of NY.

Job Overview: As an Occupational Therapist with Personal Touch Home Care, you will be responsible for assessing patients’ needs, developing and implementing treatment plans, and helping clients improve their daily living and work skills. You will also provide rehabilitative services to patients who are unable to travel to a clinic or hospital. This is focused on improving patients’ quality of life by promoting mobility, function, and independence within the comfort and convenience of their own homes.

Responsibilities:

1. Responsible for the assessment of referred patients that require rehabilitation services.
2. Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
3. Establish and administer a treatment program with specific goals determined according to the patient’s capacity and tolerance under the direction of the Physician.
4. Set realistic and achievable goals for their patients.
5. Document and record the patients’ condition.
6. Educate patients and families in an appropriate therapy method.
7. Evaluations, weekly progress notes, recertification and discharge documentation.
8. Collaborates with all disciplines to provide the best patient centered treatment and care.
9. Perform all other duties assigned by Director of Rehabilitation.
10. Compliance with confidentiality, HIPPA and Federal Residents Rights.

Qualifications:

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Demonstrates good verbal and written communication and organization skills.
• Prefer two (2) years of appropriate experience as an occupational therapist. Home health experience is preferred.
