Job summary
Evolutionary Healthcare is hiring a Nurse Practitioner or Physician Assistant to provide primary care in skilled nursing and post-acute facilities in Clearwater and surrounding areas. This role focuses on hands-on patient care, pain management, and working closely with Osteopathic Physicians to enhance patient mobility and quality of life.

Qualifications
- Licensed Nurse Practitioner or Physician Assistant in Florida (or ability to obtain).
- National Board Certification.
- DEA license (or ability to obtain).
- Experience in Internal Medicine, Family Medicine, Geriatrics, Hospital Medicine, or Post−Acute Care preferred.
Responsibilities
- Provide comprehensive primary care services in skilled nursing and post−acute care facilities.
- Evaluate patients experiencing musculoskeletal pain, mobility limitations, balance disorders, and functional decline.
- Manage acute and chronic medical conditions that affect rehabilitation and recovery.
- Educate patients and families regarding treatment plans and preventive care.
