Advocate Health is seeking an Advanced Practice Clinician to join the Orthopedics department in Mount Pleasant, Wisconsin. The role is full-time, Monday through Friday, 8:00 a.m. to 4:30 p.m., with 40 hours per week in both clinic and inpatient settings.

Responsibilities

• Evaluates, diagnoses, treats, and manages acute, chronic and complex patients in outpatient clinic and inpatient hospital settings.
• Obtains histories, conducts exams, prescribes therapies and performs minor procedures.
• Coordinates pre-operative plans, discharge planning, referrals and use of relevant medical devices.
• Prescribes pharmacologic and non-pharmacologic therapies across multiple care settings.
• Maintains patient care plans and records; provides education on health and wellness.
