Job summary

Advocate Health seeks a Physical Therapist for outpatient rehabilitation in Menomonee Falls, WI. This full-time orthopedics-focused role involves evaluating referrals, planning and delivering individualized care, and coordinating discharge and home programs.

Applicants must have a Wisconsin PT license and BLS within 6 months, with strong communication and organizational skills. The position includes a weekend rotation and a $5,000 sign-on bonus where applicable.

Qualifications

• Bachelor's Degree in Physical Therapy required.
• Wisconsin Physical Therapist license required.
• BLS certification within 6 months unless exempt,"
• Strong communication and interpersonal skills.

Responsibilities

• Interpret referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations.
• Establish a plan of care and implement treatment based on evaluation results and goals.
• Monitor outcomes and adjust treatment plans and provide status updates to physicians.
• Complete discharge planning including home programs and patient education.
• Document progress notes, billing, scheduling and adhere to insurance guidelines.
• Maintain a clean, safe, and orderly treatment area and assist with equipment.
• Demonstrate knowledge of modalities and exercise and apply appropriate evidence-based techniques.
• Participate in performance improvement activities and staff orientation.
• Provide age-appropriate care and maintain growth and development considerations.
